Addendum—ABVP Residency Logs

 

1. Running total of cases. Number consecutively throughout the program.

2. Date of assignment or examination.

3. Case number. Case number assigned by the practice/institution.

4. Species. Type full name.

5. Diagnosis. Write out, explain any abbreviations.

6. Surgical/Medical Procedure S/M + brief description of procedure or problem. Explain any abbreviations.

7. Primary or Assistant P/A. State whether you were the primary care-giver or whether you assisted the primary person.

8. Elective or emergency E/M. State whether the case was an elective case or whether it was an emergency.

9. Diplomate present or not D/N. Please indicate if an ABVP or other approved diplomate was present and advisory to you on the case.

10. Core curricular code or systems code. Please code each case using the following codes. Coding each case will facilitate summarization of case types for semi-annual report.

  • Cardiovascular—CV
  • Endocrine—EN
  • Ophthalmic—OP
  • Integumentary—IT
  • Musculoskeletalv—MS
  • Respiratory—RS
  • Gastrointestinal—GI
  • Reproductive—RP
